The high cost and limited availability of injectable GLP-1 agonists have prevented many patients from starting or continuing their therapy without interruptions. These medications have significant potential to reduce A1C levels and support weight loss.


Ewing Pharmacy offers compounded GLP-1 options, including an injectable form sourced from a 503b compounding pharmacy and a sublingual (under the tongue) form prepared in our own compounding lab.


Please note that a prescription is required to dispense this medication.

Sublingual Semaglutide?

We source the semaglutide ingredient for our sublingual compound exclusively from FDA-registered sources to ensure the highest potency and purity of our compounded suspensions.


To achieve optimal absorption through the sublingual cavity, we use a specialized sublingual base suspension. This base is formulated to carry high molecular weight drugs, like semaglutide, effectively through the sublingual cavity.

Compounded Semaglutide for Sublingual Administration

The following protocol is to help guide therapy. There is a wide variation on patients response to semaglutide. There are also inter-patient variables that may influence the absorption of semaglutide via sublingual administration

Once daily administration. Hold under the tongue for at least 2 minutes before swallowing. Do not rinse or eat/drink for 30 min.

Recommended dosage schedule for GLP naïve patients:


  • 2mg/ml: 0.5ml (1mg) once daily for 2 weeks then increase to 1ml (2mg)
  • Assess patient for dosage increase
  • 3mg/ml: take 1ml once daily
  • Assess patient for dosage increase
  • 5mg/ml: take 1ml once daily
  • Assess patient for dosage increase
  • 7mg/ml: take 1ml once daily

 

If patient has been on GLP-1 medication recently:

  • Suggested to start at 3 to 5mg daily depending on strength and duration of previous GLP-1


Injectable Semaglutide?

We source our compounded injectable semaglutide from a 503B compounding pharmacy.


A 503B pharmacy is registered with the FDA as a compounding outsourcing facility. They operate on a large scale to make the compounds available to other pharmacies. Note that the individual compounds are not FDA approved.

Compounded Semaglutide for Injection

This medication must be drawn up into syringe from a multi dose vial by patient for each dose. The syringes used are insulin syringes so the volume is converted into “Units” to be easily read. Recommend sublingual route if patient is unable to manipulate syringes or vision issues that would make reading volumes difficult.

Recommended dosage:

  • Concentration of 2.5mg/ml
  • 0.25mg (10 Units) once weekly x 4 weeks
  • 0.5mg (20 Units) once weekly x 4 weeks
  • 1mg (40 Units) once weekly x 4 weeks
  • 1.75mg (70 Units) once weekly x 4 weeks
  • Concentration of 5mg/ml—only available for 2.25mg dosage
  • 2.25mg (45 Units) once weekly for maintenance

 

Vials may be used up to the By Use Date (BUD) if refrigerated. (Stability only verified for room temperatures for 2 weeks)
